Event description

Employee partially amputates finger while operating notcher

Investigation abstract

At approximately 12:10 p.m. on November 26, 2021, an employee was operating the CMM Notcher machine. A small piece of the notched vinyl was stuck in the notcher. The employee reached inside the point of operation, and his hand was caught by the notcher when the foot pedal was activated. The eployee partial amputated of a finger and was hospitalized to treat these injuries.
