Employee amputates fingertips while operating tube bending m

Investigation abstract

At 1:00 a.m. on June 21, 2021, an employee was operating a Versa Leg Bender in the Versa Fabrication Department. While setting up the machine for a product change, the employee removed the guard and ran several test pieces by holding down a pressure sensor with their left hand. On the fourth test piece, the machine actuated unexpectedly while the employee's hand was in the pinch point, resulting in the amputation of three fingertips. The employee was hospitalized for treatment.
