Employee with COVID-19 infection dies
Investigation abstract
An employee was participating in an annual hearing conservation program Coworker #2 and all site coworkers wore face coverings during testing, which took 10 to 15 minutes each. On October 31, 2021, Coworker #1 and Coworker #2 tested positive for COVID-19. On November 04, 2021, the employee tested positive for COVID-19 , was hospitalized on November 14, 2021, and died and December 02, 2021, due to complications from COVID-19. audiogram. On October 28, 2021, an outside contractor was hired to administer the hearing conservation program audiograms to site employees in a mobile test trailer. There were 2 coworkers (both contractor employees) administering the audiograms. Coworker #1 was inside the building administering the medical evaluation questions, limiting it to 5 site coworkers at a time, to allow for social distancing. The site coworkers would then go to the test trailer where Coworker #2 was administering the audiograms. Coworker #2 was in a separate room from the site coworkers, who were tested inside a booth.
