EMPLOYEE DIES OF ASPHYXIA AFTER ENTERING CONFINED SPACE

Investigation abstract

At 9:30 a.m. on December 6. 2021, an employee was waiting for a gas monitor befo re entering a retort furnace being installed. The furnace is the permit-required confined space due to the potential for the prescence of a hazardous atmosphere . The narrative does not indicate if the employee received the meter or entered the space without testing, but he did enter when the atmosphere was oxygen defic ient, and he died of asphyxia as a result.
