Employee with COVID-19 infection dies
Investigation abstract
An employee had been working for a pharmaceutical manufacturer. The employee for work and had not called in. The emergency contact stated that the employee had the flu. The employer then notified the contact that the employee should quarantine and be tested since two coworkers who worked in his department had tested positive for COVID-19 and had worked on May 28, 2021. On June 10, 2021, the employer was notified by the emergency contact that the employee tested positive for COVID-19 and was hospitalized. On June 28, 2021, the emergency contact notified the employer that the employee died during hospitalization. The employer did not have an effective COVID-19 Prevention Program in place. Employees were observed working on production lines and in offices less than six feet apart without any engineering controls in place. Employees were observed worked as an operator on the Liquid Packaging 2 line assisting with the wearing face masks incorrectly and, in some cases, not at all throughout facility. manufacturing and packaging of products. The employee worked for the company for 12 years prior to testing positive for COVID-19. The employee worked on the first shift Monday through Friday from 5:00 a.m. to 2:00 p.m. Three coworkers working on the first shift also tested positive for COVID-19 within the same time frame. On May 28, 2021, the employee reported he began to experience COVID-19 related symptoms while off work. On June 4, 2021, the employer contacted the employee's emergency contact because the employee had not shown up
