Event description

Employee partially amputates finger when platen closes

Investigation abstract

At 5:00 a.m. on November 13, 2021, an employee noticed that blocks were not being filled completely on the Besser machine and suspected that it was a result of foreign material. The employee's hand was in the pinch point area, holding a hammer. The employee did not stop, disengage or lockout the machine. The machine cycled and the platen closed on the employee's hand, partially amputating the right middle finger at the second knuckle. The employee was hospitalized.
