Event description
Two Employees with COVID-19 infection die
Investigation abstract
An outbreak of COVID-19 was reported at a the warehouse of a freight business. Each employee had close contact with the Coworker #1 prior to becoming symptomatic. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. Coworker #1 took personal leave to travel domestically and when he returned, he worked one and a half days prior to developing COVID-19 related symptoms at which point the employer sent him home. Coworker #2 began displaying symptoms and tested positive for COVID-19 approximately one week later. Coworker #3 then became symptomatic on July 23, 2021 and was hospitalized on July 26, 2021 until August 2, 2021. Employee #1 became symptomatic July 28, 2021, tested positive on July 29, 2021, and died on August 12, 2021. Employee #2 tested positive on August 1, 2021, was hospitalized on August 8, 2021 and died on August 27, 2021.
