Employee?s right index fingertip is amputated while removing
Investigation abstract
At 1:00 p.m. on July 22, 2021, Employee #1 was attempting to remove debris from a dough cutting machine when his right hand contacted the machine's cutting blades. The employee's right index fingertip was amputated as a result of the incident. Emergency services transported the employee to a medical facility, where he was treated and hospitalized.
