Employee fractures back in fall when defective ladder slips
Investigation abstract
At 11:00 a.m. on December 9, 2021, an employee was working from a 24-foot extension ladder cleaning an elevated walking/working surface when he attempted to climb down to the ground. The ladder was missing a non-slip rubber foot pad, which allowed the ladder to kick out from under the employee. The employee fell approximately 10 feet to the concrete ground. A coworker (Foreman) called 911 and the employee was transported to a local hospital and admitted. The employee fractured his vertebrae from blunt force contact with the concrete floor.
