Event description
Five Employees test positive for COVID-19 infection
Investigation abstract
On December 2, 2021, five employees were had tested positive for COVID-19 infection. Employee #1 started having COVID-19 related symptoms on November 26, 2021. The employee does not know where acquired his infection. Employee #2 believes he acquired his infection from work. Employee #3 also believes to have acquired his infection from work. All employees are vaccinated and face coverings were enforced. No information in the narrative was provided for the other two employees. Employees had been performing normal duties in a full service restaurant. The employer did not have an effective COVID-19 Prevention Program in place. None of the employees were hospitalized.
