Investigation abstract

At 10:00 a.m. on December 15, 2021, an employee worked to remove nuts from a bolt embedded on the top of a basement wall to prepare for a sill plate installation. As the employee stepped across the corner of the wall, the employee fell approximately eight and a half feet. The employee was hospitalized to treat head injuries.
