Investigation abstract

At 2:45 a.m. on July 19, 2021, Employee #1, a 27-year-old maintenance mechanic, was inspecting a misaligned belt on an automated conveyor. While checking the belt tension with a screwdriver, his right hand was pulled into the fully energized conveyor, pinching his finger. Employee #1 sustained an amputation of the distal phalanx of his right index finger. He was hospitalized for treatment.
