Employee with COVID-19 infection dies
Investigation abstract
An employee had been fixing telecom lines. On October 26, 2021, the employee and October 31, 2021, which was negative, so he returned to work and reported symptoms on November 4, 2021, when he quarantined. Coworker #1 was fully vaccinated as of April 1, 2021, but his symptoms worsened on November 8, 2022, so he tested on November 9, 2021, which was positive. The employee had symptoms on October 30, 2021, and tested positive on November 1, 2021. On November 5, 2021, the employee was hospitalized, reported his status to his employer November 8, 2021, and remained there until he died at 2:53 p.m. on November 9, 2021. The employee was not vaccinated. Coworker #1 may have contracted COVID-19 from a customer and transferred it to others when they ate dinner together since that is the only time he had contact with the employee. Coworker #2 and Coworker 49 coworkers arrived in their own trucks at a staging area to assist a #3 also contracted COVID-19 most likely from Coworker #1. On October 20, 2021, Coworker #2 became ill and tested positive. Coworker #3 stated he had symptoms on November 5, 2021, but tested positive. Coworker #2 did not reveal his vaccination status and his employer reported no record of vaccination. Coworker #3 was fully vaccinated by August 18, 2021. The employer did not have an effective COVID-19 Prevention Program in place. short-staffed utility perform electrical repairs following a storm for 4 days. On the night of October 27, 2021, the employee (Lineman), Coworker #1 (Foreman), Coworker #2 (Lineman) and Coworker #3 (Foreman), ate together. Coworker #1 supervised a crew which included Coworker #2. On October 28, 2021, Coworker #1 began having symptoms after his shift. On October 29, 2021, Coworker #1 showed up at the staging area, but then drove back to San Diego. The rest of the crew drove back to San Diego that same day. Coworker #1 took a COVID-19 test on
