Event description

Employee sustains finger avulsion while assembling end-of-ar

Investigation abstract

At 5:25 p.m. on July 17, 2021, Employee #1, a maintenance technician at a facility that manufactures plastic municipal residential garbage cans, was building a new end-of-arm device on a workbench in the maintenance shop area. The device was not attached to any machine or connected to air or other energy sources. While manually adjusting the cylinders to position them correctly, Employee #1 pinched the tip of his left ring finger between the cylinder head and the mounting plate. Employee #1 sustained an avulsion to the tip of the left ring finger and was treated and released from Adventist Health Lodi Memorial without hospitalization.
