Employee with COVID-19 infection is hospitalized
Investigation abstract
An employee worked for a carbonated water and other beverages manufacturer. The positive for COVID-19. At 9:30 a.m. on June 29, 2021, the employee was hospitalized for a respiratory illness and treated on the hospital's ICU unit. The employee recovered and was discharged 6 days after his admission. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. employee second shift from 4:30 p.m. - 3:00 a.m. as a forklift driver removing packaged product from the production line and moving the packaged product throughout the facility. On July 3, 2021, the employee was hospitalized. EE #1. The employee had limited contact with coworkers throughout the facility, however he shared a forklift with coworkers and used common areas such as restrooms, locker rooms and break rooms. On June 21, 2021, the employee worked his last full day. On June 22, 2021, the employee reported to work and failed the entry health screening and was sent home. On June 23, 2021, the employee tested
