Event description
Four employees with COVID-19 infections die
Investigation abstract
Employees were working in an open production area at a plastics manufacturing Employee #3 worked on line 18 in the extrusion department. Employee #3 began having COVID-19 symptoms on August 18, 2021 and he tested positive on August 21, 2021. Contact tracing identified two close contacts in the same department. Employee #3 died on September 22, 2021. Employee #4 also contracted COVID-19 and died on September 20, 2021. facility and were exposed to coworkers infected with COVID-19. Employee #1 worked on line 26 in the converting department and began having COVID-19 symptoms on August 29, 2021 and tested positive the same day. Contact tracing did not identify any other employee as a close contact. Employee #1 died on September 15, 2021. Employee #2 worked in the office and on line 15 in the extrusion department. Employee #2 began having COVID-19 symptoms on August 23, 2021, and he tested positive on August 25, 2021 Contact tracing did not identify any other employee as a close contact. Employee #2 died on September 21, 2021.
