Employee amputates fingers when pulled into unguarded roll
Investigation abstract
At 4:15 p.m. on July 26, 2021, Employee # 1 was fulfilling an order for a box of plumbers tape. He was wearing cloth gloves with rubberized undersides. Employee # 1 was having difficulty with the way the machine was punching holes and the tension on the metal tape. The plumber's tape had inconsistent holes in the finished product. Employee # 1 had made some recent adjustments and was focused on that area of the machine, and not on where his hands were placed. Employee # 1 had accidental contact with the plumbers tape with his cloth glove, which snagged and pulled his hand into the unguarded roll. Employee # 1 sustained traumatic amputation of his fingers. He was hospitalized.
