Employee injures head in fall from platform to floor

Investigation abstract

At 9:00 a.m. on January 28, 2022, an employee was standing on an elevated platform kneeling on both knees while tightening a bolt on the side of a blender machine with a wrench. The employee leaned forward to turn the bolt and lost his balance, rolled to the right and fell off the platform through the guardrails. The employee incurred a head injury and was hospitalized.
