Investigation abstract

At approximately 6:15 a.m. on August 6, 2021, at Century Precision Engineering Inc., a CNC Machine Helper (Employee #1) began his shift and attempted to warm up a Doosan Puma GT2600 CNC Machine. Unbeknownst to him, the machine had been left with its door partially open and a 13" Diameter Titanium Ring part loaded on the chuck from the previous evening. Upon powering the machine, the part spun rapidly and was ejected through the partially open door, striking Employee #1 on the left arm and shoulder. This resulted in multiple fractures requiring immediate surgery and hospitalization. DOSH was notified of the incident on the same day, and an investigation followed.
