Employee falls from ladder, lacerates head and fractures arm
Investigation abstract
At 2:04 a.m. on January 13, 2022, Employee #1 was repairing a broken spring on an overhead door. Employee #1 was attempting to release the power arm for the garage door from the track. Employee #1 stated the power arm was sticking, and, when it released, he lost his balance and fell off of the ladder to the concrete floor. Employee #1 sustained a laceration to his head and a fracture to is left arm. The employee was hospitalized for treatment.
