Event description

Employee falls from vehicle and is killed due to head trauma

Investigation abstract

At 8:30 a.m. on February 5, 2022, an employee placed plywood boards on the top of a van to move them to a new location. The employee then got on top of the van to hold the boards down while a coworker drove to the new location. As soon as the vehicle began to move, the employee fell off the van and was killed due to a traumatic head injury.
