Employee amputates fingertip while clearing jammed machine

Investigation abstract

At approximately 10:00 a.m. on February 10, 2022, an employee attempted to clear hospitalization. meat jammed in the burger patty machine. The employee and co-worker were operating a burger patty-cutting machine (Formax #8) when the machine had been jammed with hamburger patties. The employee had been training the co-worker on machine operation. To clear the jam, Employee crawled under the machine and conveyor belt, reached up the meat chute, and placed their hand in the patty cutter area. The machine was in operation at the time, and the employee performed this action. The cutting stamp closed on the employee's hand, severing the left index fingertip. The employee was treated without
