Employee is killed in fall into pugmill
Investigation abstract
At 1:30 p.m. on March 2, 2022, an employee was climbing up an A-frame ladder in order to access the top of the pugmill in order to clean out the machinery. While attempting to clean out a pugmill, the employee fell through the open access door on top of the machinery, while the plant was in operation. The employee was killed.
