Event description
Employee incurs thumb avulsion while removing roll-off box
Investigation abstract
At 11:30 a.m. on February 7, 2022, Employee #1 got a call from a driver, bar was into place. Employee #1 lowered the roll-off box slowly once the bar was in place. As he continued to lower the roll-off box, the box moved about an 1 inch. He continued to lower the box. The bar bent inward, and his right thumb caught the edge of ware plate and the bent portion of the bar. Employee #1 was hospitalized to treat a right thumb avulsion. informing him of a roll-off box that would not come off the truck. Employee #1 drove out to the location to assist the driver. When Employee #1 arrived at the location, he attempted some procedures to break loose the roll-off box from the truck. The rail of the roll-off box was wedged on the driver side rail. Employee #1 asked the driver for a tie-down bar used for tightening tie-down straps. He placed the open end of the bar onto the corner guide plate and the top of the bar under the rails of the roll-off box. Employee #1 slowly lowered the back with the controls.
