Employee receives carbon monoxide gas overexposure

Investigation abstract

At 8:50 a.m. on December 13, 2021, an employee mixed bentonite inside a maintenance shop using two gas powered motors (Ditch Witch FM13v and Honda GX 390). At some point, the motors became clogged, and the employee attempted to clear the clogs as the motors continued to run. A coworker observed the employee acting confused and removed the employee from the shop. The employee was then hospitalized to treat for carbon monoxide gas overexposure.
