Investigation abstract

At 1:30 p.m. on January 1, 2022, an employee worked to connect a ground wire to a breaker with live four-hundred-and-eighty-volt buss bars in the panel. The gro und wire contacted a buss bar and an arc flash occurred. The employee was hospit alized to treat second and third degree burns to the employee's hands, left fore arm, and face.
