Employee with COVID-19 infection dies
Investigation abstract
An employee was employed by a public school system. The district serves return. The employee was tested on September 21, 2021, and reported a negative test result on September 22, 2021, which was later confirmed to have actually been a positive test on September 27, 2021. That same day the employer was notified by the employee's spouse that the employee had been hospitalized for issues related to COVID-19. On October 12, 2021, the employee's wife notified the employer that he had died from COVID-19 at 8:00 a.m. on September 11, 2021. No exposure determination could be made to conclude that the employee became exposed while working for the employer. The employee may have been exposed to a close relative that tested positive for COVID-19 just prior to employee becoming ill. The employer did not have an effective COVID-19 Prevention Program in approximately 860 students from preschool through high school. The employee had place. been working as a substitute teacher for approximately 5 years and had worked at multiple locations as needed included high schools, junior high schools, and elementary schools. On September 16, 2021, the employee was working at an elementary school when he first began to feel ill. On that same day, he notified a site secretary that he had not been feeling well and was not going to be able to fulfill the upcoming assignment at a high school.
