Employee crushes hand when caught between bed and printing c

Investigation abstract

At approximately 8:30 a.m. on November 29, 2021, an employee assembled boxes on ask another cworker to help him by standing at the controls to advance the printing cylinder and stop it so he could clean it safely. The employee needed to know any extension tools available to help clean the printing cylinder. The employee's hand got caught between the bed and the printing cylinder, crushing his hand. The employee was hospitalized to treat these injuries. the slitter/slotter machine, which scores the cardboard, cuts the cardboard, and has a printing cylinder to print on the boxes. After arriving at work, the employee noticed that the boxes were not coming out clean and began to clean the printing cylinder with a cloth and a cleaning agent The employee was wearing gloves and using a rag to wipe down the printing cylinder to clean it and learned how to operate the slitter/slotter and how to clean the machine. The employee said the other coworker had used his hands to wipe clean the cylinder. The employee said that because of production pressures, he could not
