Event description
Employee dies after falling while climbing broken pallet
Investigation abstract
At 5:00 a.m. on May 17, 2022, an employee was retrieving a chain of custody seal from the back of a storage trailer. The employee used a broken wooden pallet as a ladder to step up to the back of the trailer. The employee attempted to roll down the storage gate and lost his balance, falling 4 ft. 5 in. to the concrete parking lot. The employee sustained blunt force trauma injuries to his head and was transported to the hospital, where he was pronounced dead.
