Employee amputates three fingers while using unguarded machi
Investigation abstract
At 2:00 p.m. on December 20, 2021, Employee #1 was operating an Urschel dicing flowing properly to allow production to go up. Employee #1 reached underneath the unguarded Urschel dicing machine and came into contact with the blade of the machine. The employee was hospitalized to treat three amputated fingers on his right hand. machine located in Industrial Room #4 to dice dates. Apparently production was not going as planned because the dates were coming out too slowly underneath the dicing machine. Employee #1 told one of the supervisors of Sundate that the guard underneath the dicing machine needed to be removed to speed up the dates coming through the dicing machine. The supervisor contacted the owner of Sundate, who gave the order to remove the guard. The mechanic was contacted and removed the guard. Employee #1 operated the Urschel dicing machine for three days without the guard, reaching underneath to make sure that the dates were
