Event description

Employee is killed in fall while pressure washing duct

Investigation abstract

On May 24, 2022, an employee was pressure washing the interior of a large horizontal duct. The employee was wearing a harness and was attached to a self-retracting lifeline (SRL). When the employee reached a section of the duct that dumped into a large vertical air-material separator (cyclone), the cable on his SRL failed and he fell into the cyclone. The employee fell approximately 84 feet to the bottom on the cyclone and was killed.
