Employee is killed when struck by powered industrial vehicle

Investigation abstract

At 9:15 a.m. on March 11, 2024, an employee was driving onto a box truck with a powered industrial vehicle. The box truck began to roll upon contact with the powered industrial vehicle. While rolling forward, the box truck took the powered industrial vehicle with it, and the operator jumped and fell beneath. As the truck continued forward, the powered industrial vehicle fell, striking the operator. The employee was killed due to crushing injuries to the head and chest.
