Event description
Employee is killed when shot while on delivery route
Investigation abstract
At 3:00 p.m. on May 16, 2024, Employee #1, employed by a national package beside the van and shot him multiple times before fleeing the scene. The employee died at the scene, and his body was taken by the OC Coroner. The employer has a workplace violence policy and regularly communicates safety incidents to employees. Following the inspection, the employer received a 170B notice with no accident-related citations. delivery service, was in his delivery vehicle on his scheduled route when he was shot by an assailant. The Division was notified of the incident by the employer at 4:48 p.m., meeting the �342 reporting requirement. An inspection was opened the following morning on May 17, 2024. The employee's supervisor, who was not present during the incident, reported that the employee had stopped his vehicle at a normal location to make a delivery.
