Investigation abstract

At approximately 2:30 p.m. on June 29, 2024, an employee was performing maintenance and repair on an ultra-high pressure blasting wand. While observing the nozzle of the blasting wand to identify the defective tip, the employee stood close to the operator. The operator lost control of the wand, causing it to strike the employee in the neck. The employee sustained a fatal laceration to the neck from the high-pressure water, which was operating at 45,000 psi.
