Employee fall from same elevation

Investigation abstract

The employee feigned a fall to provoke a reaction from a coworker, dropping from was transported to the hospital, where he died from head injuries on May 30, 2024. Three additional employees were hospitalized for injuries, and three employees were treated without hospitalization. a standing position to the floor and landing on his stomach. The employee eventually died as a result of the injuries sustained from the fall. Employee #1 had parked. Wooden reels were constantly being loaded and unloaded via forklifts. Employee #1 exited his truck to attach the truck to the trailer. Employee #2, a forklift operator, came over to move the reels to a different trailer. As Employee #2 positioned the forks to lift the reels, it caused an adjacent stack of reels to fall. The forklift operator immediately jumped off the forklift to aid Employee #1.
