Investigation abstract

At 4:30 p.m. on February 12, 2025, Employee #1 was working as a pipefitter at an excavation site. The employee was using a shovel to address a water source. A frozen slab of soil became loose from the concrete wall and fell on the worker. The employee sustained fatal injuries.
