Event description

Employee dies after being struck by forklift

Investigation abstract

Employee #1, who was followed by another employee, had walked out of a lunch roo m through an aisle and entered the travel path of a forklift carrying a 24 ft wi de and 30 in. high lumber stack. When Employee #1 realized that he was being str uck by the forklift he tried to hold on to the lumber but was thrown 20 to 30 ft onto an asphalt road. Employee #1 died of head injuries in the hospital after 1 8 days of hospitalization.
