Event description
Employee killed after running into forklift
Investigation abstract
Employee #1 left his work position and was running (semi-jogging) west in the wa rehouse. At the same time, a forklift was backing up in the aisle to the north o f the warehouse to pick up a loaded pallet. Employee #1 had his head turned nort heast toward the work station and was still running when he ran into the left si de of the forklift. He fell backward, hitting his head against the concrete floo r. The forklift driver had tried to turn north toward the dryer to avoid hitting Employee #1, but the employee did not see the forklift. Preliminary reports fro m the medical examiner indicated that Employee #1 died of head injuries.
