EMPLOYEE #1 WALKED BETWEEN TWO STORAGE BINS INTO A WAREHOUSE AISLE AND WAS STRUC K BY A CLARK LIFT ORDER PICKER. THE BACK WHEEL RAN OVER HER RIGHT FOOT AND LEG. THE LEG WAS LATER AMPUTATED IN A HOSPITAL.
