EMPLOYEE #1 WAS WORKING IN A TRENCH THAT HAD NOT BEEN SHORED OR BRACED. A PIECE OF THE SOUTH WALL BROKE OFF AND CRUSHED HIM AGAINST THE NORTH WALL.
