EMPLOYEE FELL, THEN STRUCK AND CRUSHED BY FALLING MATERIAL

Investigation abstract

EMPLOYEE #1 WAS RECEIVING A LOAD OF METAL DECKING FROM THE LOAD LINE OF A CRANE. THE DECKING WAS PLACED ON TOP OF UNSECURED BAR JOISTS. THE EMPLOYEE UNHOOKED ON E CHOKER AND WENT TO UNHOOK THE OTHER SIDE. THE BAR JOISTS SUDDENLY ROLLED TO ON E SIDE AND COLLAPSED. THE EMPLOYEE FELL APPROXIMATELY 22 FEET TO THE GROUND. THE BAR JOISTS AND STACK OF METAL DECKING FELL ONTO THE EMPLOYEE, CRUSHING HIM. DEA TH WAS CAUSED BY MULTIPLE SKULL FRACTURES.
