EMPLOYEE #1 WAS USING A 12 FOOT EXTENSION LADDER TO ADJUST SOME TEMPERATURE CONT ROLS WHICH WERE LOCATED IN THE SKYLIGHTS IN THE ATRIUM ABOVE THE PLANTER. FOR UN KNOWN REASONS HE FELL AND WAS KILLED.
