Employee killed when crushed by dumbwaiter car
Investigation abstract
At approximately 9:00 p.m. on February 3, 1989, Employee #1, age 18, who had bee n a meat clerk for 5 months, was attempting to operate a dumbwaiter. The mechani cal door interlocks had been overridden with wooden blocks, allowing the dumbwai ter to operate with the hoistway doors opened. Employee #1 enlisted the help of a coworker, who pushed the basement control button, sending Employee #1 down. Wh en he arrived at the basement floor, he apparently exited from the dumbwaiter, p ushed the first floor button, and attempted to reenter the dumbwaiter. He was pi nned between the dumbwaiter car and the top of the hoistway opening and killed.
