Employees fingers amputated in power press

Investigation abstract

Employee #1 was operating a Perkins full-revolution power press, model #550-B, i n single stroke operation. The employee had just cut a piece of work and reached into the point of operation to remove it when the spring on the single stroke m echanism broke and the press cycled repeatedly. Employee #1 sustained the amputa tion of four right fingers. She was not using hand tools to remove the piece.
