Employee's fingers amputated by shaper blade
Investigation abstract
At approximately 12:20 p.m. on January 20, 1988, Employee #1 was operating a Roc kwell shaper, #1228304, to shape wood for chair frames. His left hand came in co ntact with the shaper blades, partially amputating his left index, middle, and r ing fingers. It is unknown as to how Employee #1 came in contact with the shaper blade because the machine is adequately guarded. There were no witnesses.
