EMPLOYEE #1 WAS USING A TABLE SAW TO CUT A SLOT IN A PIECE OF WOOD. THE SAW BLAD E FLIPPED THE PIECE OF WOOD UP, CAUSING THE EMPLOYEE'S FINGERS TO COME INTO CONT ACT WITH THE REVOLVING SAW BLADE.
