THUMB TIP AMPUTATED WHEN CAUGHT IN BELT

Investigation abstract

EMPLOYEE #1'S CLOTHES BECAME CAUGHT IN A PROTRUDING BELT. HIS LEFT THUMB THEN BE CAME CAUGHT AND WAS AMPUTATED AT THE FIRST KNUCKLE. THE BELT DRIVE WAS NOT GUARD ED.
