Investigation abstract

EMPLOYEE #1 WAS REPAIRING A SCREW MACHINE. TO HELP IN LOOSENING A NUT, HE HAD AN OTHER EMPLOYEE "JOG" THE MACHINE. THIS CAUSED THE MIDDLE FINGER OF HIS LEFT HAND TO BE CAUGHT IN THE SCREW MACHINE AND AMPUTATED.
