AN EMPLOYEE WAS ELECTROCUTED WHILE WORKING ON A 600-VOLT SAFETY SWITCH. HE HAD N OT LOCKED OUT THE ELECTRIC SUPPLY FOR THE SWITCH BEFORE HE STARTED WORKING.
