EMPLOYEE STRUCK BY FALLING BUILDING STONE

Investigation abstract

AT 7:40 PM ON AUGUST 22, 1986, EMPLOYEE #1, A LABORER, WAS SERIOUSLY INJURED WHE N A PIECE OF SHALL ESTIMATED TO WEIGH 300 POUNDS FELL ON HIM FROM THE BROW OF TH E TUNNEL PORTAL IN SHAFT #7. THE STONE BENT THE EMPLOYEE OVER AND SLAMMED HIS HE AD INTO THE ROAD HEADER HE WAS OPERATING. HE WAS ALSO KNOCKED OFF THE MACHINE. A S HE WAS LYING ON THE GROUND HE WAS STRUCK BY A SECOND STONE, WHICH CRUSHED HIS HIP, PELVIS AND ANKLE. OTHER INJURIES CONSISTED OF HEAD TRAUMA, CRACKED VERTABRA AND THREE VERTABRA WHICH BECAME FUSED BY THE IMPACT ON HIS HEAD.
